Dr. Miguel Delgado, M.D. presents a 6 part video where he performs a double incision mastectomy with a free nipple graft. Dave, a body builder, has severe gynecomastia due to his steroid use. Dave’s Steroid Use and His Story

In Dave’s story, he tells us he had been on steroids a long time before he had any development of male breasts. He did not notice any changes in his breasts until he stopped the steroids. He tells Dr. Delgado that he did not take any anti-estrogen as he was unaware of steroid side effects.

After he weaned himself off steroids and losing some weight, he became aware of his breast development. He was hoping that if he lost weight his male breasts would also reduce but found that was not the case, he learned that only the fat around the breast tissue would increase or decrease but not the breast tissue. The breast tissue is permanent, and surgery is the only way to get rid of it.

Confidence Lost

Dave tells Dr. Delgado how he lost his confidence as a professional athlete and trainer, once he developed breasts. He comes to Dr. Delgado for help so he can regain his confidence and be able to wear tight fitting shirts again.

Dr. Delgado explains that with surgery not all of the breast tissue is removed in order to have an aesthetically sculpted chest. Dr. Delgado warns Dave that if he should continue with steroids, it is possible that he could have a recurrence of breast tissue although not to the extent he now has. Dave states that he wishes to continue in moderation with steroids because as a man in his 50’s it is difficult to maintain his muscles in the way he wants. Dr. Delgado tells him that some men can take steroids with no adverse effects while others can take small amounts and develop breasts.

This is especially true for gynecomastia, as men are usually reluctant to talk about their “man boobs.” However, a Google search will show there are many men willing to share their experience in the hopes that they can offer support and encouragement to other men who suffer in silence with gynecomastia.

Lost Weight but Still Has Gynecomastia

For example, there is a posting by a man who tells us how his female type breasts never went away since he was a young man; no matter how much he exercised and how much weight he lost. In fact, he felt his breasts were even more visible as his chest muscles developed when he was working out. He shared how hard it was for him to see a specialist and discuss his condition, but was relieved to find how supportive the doctor and staff were. After he had done a lot of research online, he decided to proceed with surgery, even though he was very anxious. You can read reviews, read blogs, research multiple websites, but getting the story of what gynecomastia surgery is like straight from a patient is incomparable! An out of town gynecomastia patient of Miguel Delgado, M.D.’s did just that. The patient documented his surgery experience from his arrival to the hotel through to the third day after surgery.

Our patient starts his video with his arrival to his hotel and is describing how he is feeling. Since his surgery is scheduled for later in the day, what seems to be causing him the most anxiety is being hungry! All surgeries that are performed under general anesthesia need to have the patient NPO, which is a Latin term “nil per os” and means nothing to eat or drink by mouth for several hours before surgery to ensure that the patient has an empty stomach. The anesthesiologist will call the patient the day before surgery and review all instructions.

Anxious but Happy!

Our gynecomastia patient describes that besides being hungry, he is anxious and nervous but very happy that he is finally having male breast reduction surgery so that he can get on with his life.

The next topic the patient presents is reviewing all the types of medication that he will be taking. All of the pill bottles are lined up as he describes them. First are his VitaMedica vitamins that he started two weeks before surgery, then he shows his antibiotics and anti-nausea pills that are started just prior surgery and then his pain pills that are for after surgery. Most patients find they only need the pain pills for a day or two.

After taking a shower our patient heads to the surgery center and after his pre-operative preparations he is wheeled into the operating room.

Ready to Live Life

After three days we see our patient again when he is ready to get his drains removed, he is very happy and says he is glad he had his surgery and is ready to live life!

Suffering in Silence

The legend on ice developed male breasts in his late teens, as do many males; however unlike most teenage boys his never went away. He states that he didn’t spend time on the beach with his kids or his grand-kids, doing everything he could to hide his condition by not removing his shirt. He hasn’t owned a bathing suit in more than 40 years! He decided he had enough when he was too embarrassed to remove his shirt while out on his boat. This cannot be emphasized enough; there are many men that have disappointing results when they do not choose a surgeon with vast experience. If your surgeon tells you that liposuction alone will get rid of your gynecomastia breast, get another opinion or even two. “Pseudo-gynecomastia” is caused by fat alone and is the only condition that can get a good result from just liposuction. Only a very small percentage of men have gynecomastia caused by fat only. True gynecomastia needs to have a combination of liposuction and surgical removal of breast tissue.

Gynecomastia Treatment: Psychological VS Physical

Most men that suffer with gynecomastia, had it develop when they reached puberty. Puberty is an extremely sensitive time for young men without the addition of the gynecomastia breast. A few young men are able to deal with the condition without it impairing their life style, they go to the beach, and take their shirts off for gym. Although, they almost always have to deflect cruel remarks until such time that the condition resolves itself or they seek gynecomastia treatment. But the majority of young men with the male breast suffer silently, most are too embarrassed to even get support from their parents. They refuse to remove their shirts, and they learn to stand with their shoulders rolled forward. Unfortunately, years of this type of behavior can cause great damage to one’s psyche. It is not unusual for young men to become obsessed with their condition and find that they can blame almost all of their problems on the gynecomastia breast.
At some point in their adult life, many men are able to get gynecomastia treatment, which involves the surgical removal of most of the male breast tissue and fat. For most men this is a life altering experience, the relief and immediate joy is profound. But for a very few, there can be some disappointment that not all of their problems in life have disappeared. They may have an unrealistic expectation of what their results should be, and find almost an obsession for constant minor revisions.  Dr. Delgado stresses that this is a very small minority, and these men would most likely benefit from psychological counseling.
